Reasonable Workplace Accommodation Request



A reasonable workplace accommodation request may significantly enhance employee performance by addressing the unique needs of individuals with psychiatric disabilities. These accommodations are designed to mitigate functional limitations that may hinder an employee’s ability to execute essential job duties effectively. For example, modifications such as flexible work hours or a quieter workspace can help reduce anxiety or distractions, enabling employees to focus better and perform at their best. Ultimately, it is the employer’s responsibility to approve these requests.

1. Schedule an appointment today for an evaluation to assess your reasonable workplace accommodation needs.

2. Upload a copy of your functional job duties, any relevant mental health records or documentation, and any specific forms that need to be completed from your place of employment.

3. Complete Workplace Assessment Questionnaire and associated consent forms 
Your appointment will then be schedule. If we have any questions or need clarification, we will contact you prior to your appointment. 

4. You will receive a copy of your workplace accommodation request within 1-2 business days from the evaluation date.

FMLA Requests



The Family and Medical Leave Act (FMLA) provides eligible employees with unpaid, job-protected leave for certain health conditions, such as a serious mental health condition. FMLA leave can be taken continuously (in one block of time) or intermittently (in separate blocks or as a reduced schedule).

Do you need to take off time from work and have your request approved quickly? Have you not seen a mental health provider in a while or your current provider is not available?

1. Confirm you are eligible to use FMLA with your human resources department

2. Schedule an appointment today for an evaluation to assess your FMLA request
Upload a copy of your functional job duties, any relevant mental health records or documentation, and any FMLA specific forms that may need to be completed 

3. Complete the FMLA Request Questionnaire and relevant consent forms. Upload a copy of your photo ID. If we have any questions, we will contact you prior to your appointment.  Your appointment will then be scheduled and your evaluation will be completed. 

4. You will receive a copy of your FMLA request within 1-2 business days from the evaluation date.
